The Grammy-winning singer revealed the news in an interview with The New York Times, referring to Cowan as their fiancé for the first time.
The announcement comes just ahead of the release of Smith's fifth studio album, Hazel Eyes, which is named after Cowan's eye colour and explores the couple's relationship.
According to Smith, the album's opening track, Everlasting Love, was started the night before their first date and completed after the pair had begun their relationship.
The couple first met online before making their public debut in 2023, when Cowan designed costumes for Smith's I'm Not Here To Make Friends music video.
They later appeared together at the 2024 Met Gala.
Cowan has become one of the fashion industry's rising names, creating designs for stars including Jennifer Lopez, Lady Gaga and Heidi Klum.
Smith, meanwhile, recently moved to New York, saying the city has given them the freedom to fully express themselves.
Although the singer has previously said they prefer to keep their personal life private, they have spoken warmly about the relationship while promoting their new album, describing Hazel Eyes as a romantic record inspired by love.
